United Homeless Healthcare Partners (UHHP) is pleased to offer several exciting new workshops in 2012. Each workshop series is a part of a larger initiative to increase capacity, share lessons learned and create communities of practice that examine innovative ways to serve homeless and chronically homeless individuals and families in Los Angeles County.

2012 Workshop Series - Integrating FQHC Healthcare in Permanent Supportive Housing
This series of three workshops are designed to assist Supportive/Affordable Housing Developers and FQHC’s partner together to ensure that health care services are available to individuals and families residing in supportive housing. UHHP and CSH would like to foster the development of health care/housing projects throughout Los Angeles County. We anticipate that graduates of the workshops series will have the practical tools necessary to move to the next phase of project development. Throughout the series we will encourage natural alliances and help organizations develop the tools to “operationalize” this type of project.
Who should attend this training?
Executive Management of Permanent Supportive Housing/Affordable Housing Development Agencies and Federally Qualified Health Centers that are interested in partnering together to provide physical, mental and substance abuse care to individuals and families residing in supportive/affordable housing. The series was designed particularly for organizations that are considering or are currently in the planning phases of developing a project of this type.
Cohort Design
This series is designed to encourage partnerships and invite the development of new projects in Los Angeles County. There will be significant time allotted for group work. We therefore encourage you to invite and attend the workshop series with your service partners or potential service partners and we ask that each member (individual or organization) of the cohort commit to attending the entire series of workshops in the sequence listed below.

Setting Foundations: Understanding Each Other’s Business
The goal of this workshop is to create a common language and understanding regarding business operations among both Permanent Supportive Housing Developers and Federally Qualified Health Centers (FQHC’s). This workshop will define both permanent supportive housing (PSH) and Federally Qualified Health Centers (FQHC’s), explain how financing works for both, as well as define the needs of the target populations for housing developments that seek to integrate health care, mental health care and substance abuse services.

Three Program Models in Los Angeles
The goal of this workshop is to increase awareness of the successful project models and finance structures utilized in Los Angeles County. This workshop will explore the three service delivery models currently in operation or planned in Los Angeles, including 1) Project-based services with on-site satellite clinics, 2) Mobile services, and 3) Off-site clinic-based services. We will review the advantages and disadvantages of each model, explore sample budgets and projected Medi-Cal reimbursement related to each model. Attendees will have the opportunity to work on project budgets during the workshop with feedback from facilitators

Developing Service Partnerships
The goal of this workshop is to assist potential collaborative partners “think through” the collaborative structure and mechanisms necessary to support a multi-agency, multi-service project. The topics of this workshop include, building a shared culture, developing project governance structures and designing operational agreements, planning for intra-organizational information sharing, data collection and project evaluation.
The workshop is limited to 25 attendees. Please book early to ensure a seat!
